Noah’s Bark Kennel Boarding Agreement

The pet’s “Owner” agrees to pay all costs and charges for boarding and charges for special services requested or required and all veterinary costs for pet during the time said pet is in the care of the “Kennel”: Boarding charges are per calendar day. By signing this contract and leaving the pet with “Kennel”, Owner certifies that all information given about their pet is true and accurate.

All animals admitted must be current on vaccinations: DHLPP/C, Rabies & Bordetella for dogs: FVRCP Rabies & Feline Leukemia for cats. Please bring a copy of your pet’s current vaccination record or have your vet fax directly to us. Pets must be free of external parasites: any animals found to have fleas or ticks will be bathed/treated at the “owner’s” expense.

The “Kennel” agrees to exercise due and reasonable care in the boarding of all pets and maintain the premises in clean, safe and sanitary condition. The “Owner” assumes the risk of sickness, injury or death and the “Kennel” will not be held liable. If a medical problem arises, the “kennel” will attempt to contact the “Owner” or other emergency contact to determine the course of action. If we are unable to contact the “owner” or emergency contact, every attempt will be made to contact the “owners” veterinarian, if unavailable; the “kennel” has permission to take the pet to the on-call veterinarian for diagnosis and treatment. The “owner” is personally liable for all fees for lodging, veterinary services and any other expenses incurred while the animal is boarded. “Owner will make no claim of any kind against “kennel” by reason of such sickness, injury or death of my animal(s).

The Kennel will exercise all reasonable precautions against injury, escape, accidents or death of pet. It is expressly agreed by “owner” and “kennel” that “kennel’s” liability shall not exceed the current chattel value of a pet of the same breed or the sum of $300 per animal, whichever is less. The “owner” further agrees to be solely responsible for any and all acts or behavior of said pet while in the care of the “kennel” so “owner” needs to make the “kennel” aware of any problems the pet may have (ie, excessive barking, biting, frightened of thunder etc).

 All charges incurred by Owner shall be payable upon pick up. The Kennel shall have and is hereby granted a lien on the pet for any and all unpaid charges resulting from boarding pet at kennel. The Owner hereby agrees that in the event the boarding charges are not paid when due in accordance with this contract, the Kennel may exercise its lien rights upon ten days written notice given by Kennel to Owner by certified mail to address on clients card. Kennel may dispose of pet for any unpaid charges by sale or otherwise, in the discretion of the Kennel and Owner waives all statutory or legal rights to the contrary.

Any controversy or claim arising from this contract or the breach of this contract shall be settled by arbitration in accordance with the rules of the American Arbitration Association and any judgment will be entered by any Court having jurisdiction.
